Using Google Forms to collect Data

I really like using google forms for various data gathering tasks. I hope to use them this year to collect information about how the students are doing. Here is a Google Form example that I will use with our first math unit.

To input the data I can use the spreadsheet or use my tablet or smart phone to input while walking around the room observing in the moment. Since it is a google form I can also share it with my amazing kindergarten team and they can also use it with their students too.
